1. Phone number with Area Code
2. Company Name
3. Contact name
4. Address
5. e-mail
6. Hard copy of your resale tax number
7. Picture of store, display and install bay (car stereo customers only)
8. Copy of canceled business check
9. Password of your choice (5 characters minimum)
Your username will be sent to you via e-mail.
